If you have, you are already familiar with the relatively common condition known as TMD.

TMD, short for temperomandibular disorder, is estimated to affect as much as 40% of the population at some point in their lifetimes, and is especially prevalent in the young. Initial diagnosis is typically made through a visit to the dentist, and the eventual treatment multidisciplinary with various management techniques. One of the lesser known treatment techniques, but something that is gaining in popularity, is soft-tissue therapy of the masticatory muscles the muscles used to move the jaw.

As a sports chiropractor using manual soft-tissue therapy on a daily basis, I have treated TMD on many occasions with success, however good research on the subject has been lacking. Recently however, this type of myofascial therapy for TMD was tested for efficacy in a randomized, controlled clinical trial. Researchers from Macquarie University in Sydney, Australia examined the clinical effectiveness of manual therapy in the treatment TMD. Specifically, they compared myofascial therapy to myofascial therapy plus self-care and education and no care (the control group in the study).

This was a private clinic-based study, with eligible participants ranging from age 18-50 presenting with jaw pain with or without joint sounds of at least 3 months and a willingness to participate in the study. Patients with physical or psychological contraindications were excluded. The outcome measures were jaw pain at rest, jaw pain upon maximal active opening, and jaw pain upon clenching.

The primary therapy consisted of myofascial release of the masticatory muscles, including the temporalis, medial and lateral pterygoid, and masseter. The subjects were treated twice per week over a period of 6 weeks for a total of 12 therapy sessions. Additionally, one group was given additional education and self-care consisting of self-mobilization, exercises and stretching, as well as general education on TML anatomy, biomechanics, disc displacement and dysfunction, and the role of psychoemotional factors contributing to TMD.

The results showed a statistically significant positive efficacy for both the myofascial therapy only, and the myofascial therapy plus education and self-care groups. Not surprisingly, the therapy plus self-care group showed a better 1 year outcome than the therapy alone group a trend that has been supported for the care of most musculoskeletal conditions.

Although the results of this study are no surprise to those practicing manual therapy, it serves to reinforce what we already know anecdotally. For patients suffering from TMD there is now evidence for another great intervention, and for manual therapists not currently treating TMD, there is a great opportunity to expand your skillset and serve those in need.

MSF’s own clinics are often overwhelmed by people who can’t afford to pay for private care. Public hospitals and clinics are plagued by management problems and shortages of medications, says the organization.

One thing for sure, is that the concept often feels overwhelming when we first try this type of approach.

My view point is that, guided discovery, can be one of the most beneficial interventions used within CBT. In a nut-shell, guided discovery is a process that a therapist uses to help his or her client reflect on the way that they process information. Through the processes of answering questions or reflecting on thinking processes, a range of alternative thinking is opened up for each client. This alternative thinking forms the blueprint for changing perceptions and behaviours.

Essentially, it could be suggested that a client approaching a CBT therapist is initially functioning at a mindless level. This is not to say that a client is ignorant or not able to think. Rather, that the client has a range of automatic cognitive processes that are being allowed to run without intervention. Some of these thinking processes may have been developed in childhood, while others may directly result from an immediate mood state.

A useful analogy for guided discovery is to think about going to an opticians for an eye test. The optician may initially put a contraption on their clients head with a range of lenses on it. Initially, their clients cannot see through this contraption very well (i.e., what they perceive is a blur). The optician then sets about gradually removing or replacing lenses, and after a while, through a process of trial and error (and feedback from the client) each individual begins to see more clearly.

Guided discovery works in exactly the same way as the above example. Except, instead of using optical lenses, the CBT therapist helps the client use lenses of perception. Perceiving information is a different way allows each client to access a range of choices in their life, ostensibly, to see their life through different lenses. When we view life in a different way our emotional reaction to events also shifts. These types of continued conscious re-evaluations in CBT are very important because they lay the foundations of future automatic thinking and make relapse less likely.

This is another vitamin article that I have written, but the only difference is that this one goes into more detail about specific vitamins. My previous articles do not do that. Those articles were more of a short introduction into vitamins, their role and their best food source. I’ll start with 2 water Soluble Vitamins and 1 fat soluble vitamins.

Water Soluble Vitamins: These are water-soluble vitamins that are not stored in the body and must be replaced each day. These vitamins are Vitamin B1, Vitamin B2, vitamin B3, vitamin B6, vitamin B12, Folic Aid, Pantothenic Acid, Biotin and vitamin C.

Lets start with the b vitamins: Vitamin B1 is needed to process carbs, fats, and proteins. Vitamin b1 is required by everybody to form the fuel the body runs on. Every nerve cell requires vitamin b1 to function properly. Wheat germ, peas, beans, enriched flour, fish, peanuts, and meat are all excellent sources of vitamin b1. People with advancing age, a poor diet and too much alcohol consumption may result in vitamin b1 deficiency. The suggested RDA for vitamin b1 is unclear but The amount found in most multivitamin supplements is more than enough. Vitamin b1 works hand in hand with vitamin b2 and vitamin b3.

Vitamin b2: Helps release energy from foods. Essential for healthy eyes, skin, nails and hair. Their sources are: Whole grains, brewer’s yeast, torula yeast, wheat germ, almonds, sunflower seeds,cooked leafy vegetables. The suggested RDA for vitamin b2 is unclear but The amount found in most multivitamin supplements is more than enough.

Please note, be aware that vitamin b3 or Niacin, as its commonly known as, may cause a reaction in some people. Itching and or burning are two of the symptoms. The author of this article found that out the hard way. Fat-soluble vitamins: Theses vitamins are A, D, E and K. These vitamins are dissolved in fat and stored, therefore they are not needed every day in the diet. Over consumption of fat soluble vitamins may pose a risk of toxicity.

Vitamin A: Vitamin a is a fat soluble vitamin and as mentioned before, should not be consumed in excess. For the maintenance of skin, mucous membranes, bones, teeth, and hair; eye sight. Their sources are: green vegetables, melon, squash, tomatoes.
